State disciplines local health care providers

The state Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in Kitsap.

OLYMPIA — The state Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in Kitsap.

In December, the Nursing Assistant Program charged registered nursing assistant Annette Christine Gross (NA00048531) with unprofessional conduct. Charges say the Department of Social and Health Services found that Gross, who provided residential services to vulnerable adults, exploited clients for $3,568.15.

DSHS will not allow Gross to be employed in caring for or having unsupervised access to vulnerable adults.

In November, the secretary of Health conditionally granted a registered nursing assistant credential to David Warren Mills (NA60479030) and ordered him to participate in a substance abuse monitoring program.

In 2008 and again in 2014, Mills was convicted of driving while intoxicated. Mills underwent a substance abuse evaluation that indicated he could practice with reasonable skill and safety if he follows treatment recommendations.

In November, the Medical Commission ended probation for physician Janet E. Vondran (MD00033182).

In November, the Chiropractic Commission modified an agreement with chiropractor Blake J. Wagner (CH00003095) that gives him until Sept. 16, 2015 to pay a $1,000 fine, and to complete continuing education in boundaries and ethics.

In September 2013, Wagner agreed to fulfill those requirements within one year.

Wagner had asked a patient to lend him $600, called her and went to her house.

His license remains suspended.

In October, the Nursing Assistant Program conditionally granted a certified nursing assistant credential to Christina Lee Kauffroath (NC60492009) and placed her on probation for at least one year.

In August 2014, Kauffroath was placed on probation when she was conditionally granted a registered nursing assistant license.
